The Wellness Credit Program is Sundt’s annual incentive designed to support your health while helping you save money. The program encourages employee-owners and covered spouses to get an annual physical to prioritize their well-being, help detect potential health problems early, and build relationships with healthcare providers.
Program Period:
(October 1, 2026 – September 30, 2027)
Complete an annual physical between October 1, 2026 and September 30, 2027 to earn the Wellness Credit for the 2028 plan year.
Eligibility:
Employee-owners and covered spouses enrolled in a Sundt medical plan are eligible to participate in the Wellness Credit Program. Please note, children do not need to participate.
Potential Savings:
Employee-owners save $1,800 on their medical plan premiums for 2028, and $3,600 when a covered spouse participates.
How to Participate:
- Complete your annual wellness exam. Schedule an appointment with your primary care provider (PCP) and use your Sundt medical insurance. Visits may be completed in person with a PCP or virtually through Telehealth from AZ Blue.
- Self-attest through Sharecare (Optional). Employee-owners and spouses can also self-attest to completion of their exam through Sharecare. Visit azblue.sharecare.com to log in or create a Sharecare account using your BCBS Member ID. On the home page under “Your Rewards” select “2027 Sundt Strong” and then “Wellness Credit Program (Self-Attest).”
- Allow time for claims verification. Sundt receives confirmation of appointment attendance from Blue Cross Blue Shield. After your wellness exam is validated, confirmation of your wellness credit will be visible in Sharecare under the “Wellness Credit Program (Claims-Verified)” section.
Please note: This program is participation-based only. Sundt does not receive any medical results or personal health information. We only receive confirmation that you completed your annual physical.
Know Before You Go:
- Important: It is important that employees and covered spouses create their own Sharecare accounts. Once an account is created and the appointment has been validated, Sharecare will send an email confirmation. Participants are also encouraged to complete the self-attestation as a backup.
- Your insurance covers one annual physical per calendar year, so you do not need to wait 365 days from your last exam. Exams with an in-network PCP are covered at 100% (in-person or virtually).
- Sundt receives confirmation of appointment attendance from Blue Cross Blue Shield. After your wellness visit is validated, confirmation of your wellness credit will be visible in Sharecare under the “Wellness Credit Program (Claims-Verified)” section.
