Health Care/Vision/Prescription Benefits: Since 1981, Sundt has maintained a self-insured employee welfare benefit plan to provide medical benefits to its employee-owners. Sundt has four plan options available for employee-owners and their dependents to choose from, including a newly added High Deductible Health Plan with a Health Savings Account. These plans offer a great Preferred Provider Organization (PPO) network, which allows for additional cost savings on medical claims, vision coverage, and prescription coverage. Mail order generic maintenance prescriptions are currently free to participants under the plan on select medications with 90 days fill.

Wellness Program Benefits: Sundt maintains a comprehensive wellness program SELF (Sundt Employees Living Fit), which has the following goals:
- Build a culture of wellness at Sundt
- Use the health plan as a “health plan” and not a sick plan
- Detect and manage health issues before they become serious
- Encourage individuals to take an active role in preventive care
- Help individuals get engaged and be accountable for their health
- Control rising health care costs

Health Management Program: This program promotes wellness and encourages prevention by providing free health evaluations for all employee owners and spouses/domestic partners covered under the Plan. The program’s health evaluation is comprised of three components: a health risk assessment that includes an analysis of health risks and medical history; blood pressure test; and a comprehensive blood screening. Participants who receive their health evaluation during the designated period in August/September currently receive an incentive for the following Plan year. The program provides individual health goals for participants, along with health coaching, access to a website with wellness tools, and a free midyear blood test to check on their progress in meeting their health goals.
Preventive Care: Based on guidelines by the American Medical Association, preventive, non-diagnostic care with no deductible and no annual limit. Most use the allocated funds for items like flu shots, physicals, yearly well-woman and well-man exams.
Employee Assistance Program (EAP): Confidential counseling and referral service is available to employees and their dependents at no cost, 24 hours a day, 7 days a week.
Life Insurance & Disability:
Life Insurance Benefits: Employee-owners are provided basic life insurance and accidental death and dismemberment insurance.
Disability Insurance: Sundt provides to employee-owners at no cost two disability benefit programs. Short-term disability is a salary continuation program that pays a portion of an employee’s pay while off work due to disability. Long-term disability insurance provides a disabled employee 60% of his/her gross monthly salary prior to a disability.

Paid Time Off (PTO) Program: This program includes time for vacation, sick and personal days. The number of days given to employee-owners is based on years of service with the company.

Note: Some of the benefit plans listed above have eligibility rules based on subsidiary, classification, etc. and have a waiting period prior to eligibility. There is also a 20-hour a workweek minimum requirement to be eligible for most benefits.
